1. Purpose and Scope

This report documents the monitoring, measurement, analysis and evaluation of the information security performance and the effectiveness of the Information Security Management System (ISMS) of for the period [Period Start] to [Period End]. It is produced to satisfy ISO/IEC 27001:2022 Clause 9.1 and to support the ongoing and separate evaluations of controls expected under SOC 2 CC4.1. It covers the security processes and controls in the defined ISMS scope, and provides an evidence-based input to the Management Review meeting.

2. What Is Measured

In accordance with Clause 9.1(a), the following are determined for measurement: (a) the operational security processes and their controls (patch management, incident response, vulnerability management, access governance, security awareness, and backup/recovery); (b) the ISMS information security objectives and their associated key performance indicators (KPIs); and (c) the effectiveness of the controls in achieving those objectives. Each metric has a defined data source, a target (acceptance criterion), and a designated owner. Metrics are selected so that they are relevant, quantifiable, and repeatable, rather than merely activity counts.

3. Measurement Methods

Per Clause 9.1(b), methods are chosen to produce valid, comparable and reproducible results. Quantitative metrics are computed from authoritative source systems (endpoint/patch management console, ticketing and SIEM records, vulnerability scanner, identity and access management logs, learning management system, phishing-simulation platform, and backup/restore logs). The same definition, formula, and measurement window are applied in each period so that trends over time are meaningful. Where a metric relies on sampling (for example, restore testing), the sample size and selection basis are recorded. Data is reconciled to source before inclusion, and material data-quality caveats are disclosed in the analysis.

4. When Measurement Is Performed and by Whom

Per Clause 9.1(c) and (d), metrics are collected on a quarterly basis, aligned to the reporting period, with certain operational metrics (such as incident MTTR and patch SLA) tracked continuously and aggregated for the period. is responsible for collecting and computing the metrics from the source systems. Individual control owners (IT operations, the security operations team, HR/learning, and infrastructure) are accountable for the underlying data. Timeliness of collection is itself monitored so that reporting does not slip.

5. Analysis and Evaluation Responsibility

Per Clause 9.1(e) and (f), the collected results are analysed and evaluated by following the close of each quarterly period. Analysis compares actual values against targets, examines the direction of travel against prior periods, identifies root causes for any metric below target, and evaluates whether the ISMS and its controls remain effective and are achieving the stated information security objectives. The conclusions, together with recommended corrective actions and their owners, form the analysis recorded in this report.

6. Metric Definitions

The security KPIs reported below are defined as follows: (a) Patch SLA % — the proportion of applicable security patches deployed within the defined SLA window; (b) Incident MTTR — the mean time from detection to resolution of security incidents; (c) Security training completion % — the proportion of in-scope personnel who completed mandatory awareness training in the period; (d) Vulnerability closure time — the average time to remediate critical/high findings from discovery; (e) Access-review completion % — the proportion of scheduled periodic access reviews completed and signed off; (f) Phishing fail rate — the proportion of recipients who failed a phishing simulation (lower is better); (g) Backup restore success % — the proportion of scheduled restore tests that completed successfully with data integrity confirmed.

8. Analysis and Evaluation of Results

The results above have been evaluated against their targets and against prior periods. Metrics marked “Met” indicate the associated controls are operating effectively and continue to achieve their information security objectives. Metrics marked “Watch” are within tolerance but are trending unfavourably and are being monitored more closely. Any metric marked “Below target” or “Above threshold” represents a control-effectiveness gap: a root-cause analysis has been performed, and a corrective action with an accountable owner and due date has been raised in the ISMS corrective-action log. Adverse phishing and patch results, where present, are treated as leading indicators of exposure and prioritised accordingly. This evaluation constitutes the analysis required by Clause 9.1(e)-(f) and the ongoing evaluation of control operation expected under SOC 2 CC4.1.

9. Corrective Actions and Continual Improvement

For each metric not meeting its target, a corrective action is recorded with a root cause, a remediation plan, an owner, and a target completion date, and is tracked to closure in line with Clause 10.1 (nonconformity and corrective action). Recurring shortfalls are escalated and may trigger a re-evaluation of the target, the control design, or resourcing. The effectiveness of corrective actions is confirmed in the subsequent reporting period by re-measuring the affected metric. This closes the monitor–analyse–act loop and drives continual improvement of the ISMS.

10. Reporting to Management Review

This report, together with the metrics table and analysis, is a documented input to the Management Review meeting in accordance with ISO/IEC 27001:2022 Clause 9.3. It provides management with an evidence-based view of information security performance, control effectiveness, the status of corrective actions, and any resourcing or risk-treatment decisions required. Management review outputs (decisions, actions, and any changes to objectives or targets) are recorded and fed back into the ISMS. The next Management Review meeting is scheduled for [Next Review Date].

11. Retention of Documented Information

This report and the underlying metric data are retained as documented information evidencing the results of monitoring and measurement, as required by ISO/IEC 27001:2022 Clause 9.1. Records are retained for a minimum of three years (or longer where contractual, audit, or regulatory requirements apply) and are made available to internal audit and to external assessors during ISO 27001 certification audits and SOC 2 examinations. Access to the underlying data is restricted to authorised personnel.
